APPLICATION FOR NEW PERMITS


As you well know, dealing with any entity may require specialized forms to complete your transaction and/or business; liquor licensing is no exception.

NEW LIQUOR PERMITS

APPLICATION FOR MANUFACTURER, BREWPUB OR WAREHOUSE (WITH OPTION FOR D6) PERMIT

Below are all the forms you will need to file for a class A-1, A-1-A, A-2, B-2A, S, A-3, A-4 Or W (With Option For D6) Permit. Please read all instructions carefully and if you still have questions, please call (614) 644-2411. You will need the following forms to file with a class A-1, A-1-A, A-2, A-3, or A-4 permit. You will also need to secure a "Civilian Identification Card". Please request Liquor Control to send you one by calling (614) 644-2411. Remember, your A-1, A-1-A, A-2, A-3, Or A-4 (With Option For D6) permit application cannot be approved without submission of the "Civilian Identification Card".

APPLICATION FOR CLASS "B" DISTRIBUTOR PERMIT
Below are all the forms you will need to file for a class B1, B2, B3, B4, and/or B5 Permit. You will need the following forms to file with the above class(es).

APPLICATION FOR "B-2A" PERMIT .
A B-2a permit may be issued to a wine manufacturer, a brand owner or United States importer of wine, or the designated agent of a brand owner or importer for all wine sold in Ohio for the owner or importer to sell wine to retail permit holders provided the manufacturer of the wine is entitled to a tax credit under 27 C.F.R. 24.278 and produces less than one hundred fifty thousand gallons of wine per year. APPLICATION FOR D-4 (CLUB) PERMIT

A D-4 Permit may be issued to a club which has been in existence for three years or more prior to the issuance of such permit to sell beer and any intoxicating liquor to its members only, in glass or container, for consumption on the premises where sold. The fee for this is $469.00 for each location. No such permit shall be granted or retained until all elected officers of such organization controlling such club have filed with the Division Of Liquor Control and a statement certifying that such club is operated in the interest of the membership of a reputable organization, which is maintained by a dues paying membership, setting forth the amount of initiation fee and yearly dues. The roster of membership of a D-4 permit holder shall be submitted under oath on the request of the Superintendent of Liquor Control. Any information acquired by the Superintendent or the division with respect to such membership shall not be open to public inspection or examination and may be divulged by the Superintendent and the division only in hearings before the Liquor Control Commission or in a court action in which the division or the Superintendent is named a party.

The requirement that a club shall have been in existence for three years in order to qualify the D-4 permit does not apply to units of organizations chartered by Congress or to a subsidiary unit of a national fraternal organization; provided that the parent organization has been existence for three years or more at the time application for a permit is made by such unit.

Application For New D-7 Resort Area Permit
Beer and intoxicating liquor by glass or container, for consumption on premises only, until 2:30 a.m. during the resort season.
Permit Fee: $469.00 per month each month of the resort season.
Payment for full resort season must accompany this application.

Application For "E" Permit
Section 4303.19 - Permit E may be issued to the owner or operator of any railroad, a sleeping car company operating dining cars, buffet cars, lounge cars, or similar equipment, or an airline providing charter or regularly scheduled aircraft transporation service with dining, buffet, club, lounge, or similar facilities, to sell beer or any intoxicating liquor in any such car or aircraft to bona fide passengers at retail in glass and from the container for consumption in such car or aircraft, including sale on Sunday between the hours of one p.m. and midnight.

APPLICATION FOR "G" PERMIT .
Permit G may be issued to the owner of a drugstore in charge of a registered pharmacist to be named in such permit for the sale at retail of alcohol for medicinal purposes in quantities at each sale of not more than one gallon upon the written prescription of a physician or dentist who is lawfully and regularly engaged in the practice of his/her profession in this state, and for the sale of industrial alcohol for mechanical, chemical, or scientific purposes to a person known by the seller to be engaged in such mechanical, chemical, or scientific pursuits; all subject to section 4303.34 of the Ohio Revised Code. APPLICATION FOR "H" PERMIT
TO TRANSPORT BEER, INTOXICATING LIQUOR, AND ALCOHOL

An "H" permit may be issued to a carrier by rail or motor vehicle who also holds a license issued by the public utilities commission to transport beer, intoxicating liquor, and alcohol, or any of them, in this state or delivery or use in this state. Please note that if you pick-up or deliver alcohol in the State of Ohio, you are required to apply for an "H" permit. However, if you are just passing through the State of Ohio, it is not required to file for any type of permit from the Division of Liquor Control. Complete this application to transport beer, intoxicating liquor and alcohol.

DLC4147 - Application For "H" Permit
Application for an "I" permit.
Section 4303.23 An "I" Permit may be issued to wholesale druggists to purchase alcohol from the holders of A-3 permits and to import alcohol into Ohio subject to such terms as are imposed by the division of liquor control; to sell at wholesale to physicians, dentists, druggists, veterinary surgeons, manufacturers, hospitals, infirmaries, and medical or educational institutions using such alcohol for medicinal, mechanical, chemical, or scientific purposes, and to holders of G permits for nonbeverage purposes only; and to sell alcohol at retail in total quantities at each sale of not more than one quart, upon the written prescription of a physician or dentist who is lawfully and regularly engaged in the practice of his profession in this state. The sale of alcohol under this section is subject to section 4303.34 of the Revised Code. "Wholesale druggists" as used in this section includes all persons holding federal wholesale liquor dealers' licenses and who are engaged in the sale of medicinal drugs, proprietary medicines, and surgical and medical appliances and apparatus, at wholesale. APPLICATION FOR "S" PERMIT .
An S permit may be issued to a wine manufacturer, a brand owner or United States importer of wine, or the designated agent of a brand owner or importer for all wine sold in Ohio for the owner or importer to sell wine directly to consumers via the mail provided the manufacturer of the wine is entitled to a tax credit under 27 C.F.R. 24.278 and produces less than one hundred fifty thousand gallons of wine per year.The fee for a S permit is $25.00. Duplicate Bar Application.
§ 4303.30 Duplicate permits.
The rights granted by any D-2, D-3, D-3a, D-4, D-4a, D-5, D-5a, D-5b, D-5e, D-5f, D-5g, D-5h, D-5i, D-5j, D-5k, or D-6 permit shall be exercised at not more than two fixed counters, commonly known as bars, in rooms or places on the permit premises, where beer, mixed beverages, wine, or spirituous liquor is sold to the public for consumption on the premises. For each additional fixed counter on the permit premises where those beverages are sold for consumption on the premises, the permit holder shall obtain a duplicate D-2, D-3, D-3a, D-4, D-4a, D-5, D-5a, D-5b, D-5e, D-5f, D-5g, D-5h, D-5i, D-5j, D-5k, or D-6 permit.

The holder of any D-2, D-3, D-3a, D-4, D-4a, D-5, D-5a, D-5b, D-5e, D-5f, D-5g, D-5h, D-5i, D-5j, D-5k, or D-6 permit shall be granted, upon application to the division of liquor control, a duplicate D-2, D-3, D-3a, D-4, D-4a, D-5, D-5a, D-5b, D-5e, D-5f, D-5g, D-5h, D-5i, D-5j, D-5k, or D-6 permit for each additional fixed counter on the permit premises at which beer, mixed beverages, wine, or spirituous liquor is sold for consumption on the premises, provided the application is made in the same manner as an application for an original permit. ... The application shall identify by name, or otherwise amply describe, the room or place on the premises where the duplicate permit is to be operative....

Note: The holder of a D-1, D2X, or D-3X permit is not required to obtain a duplicate bar permit if the additional bar is exclusively used for the sale of beer only, or wine only if a D-3X. An A-1-A duplicate bar permit is only required with a D6.
