Divisional Forms In Numerical Order


As you well know, dealing with any entity may require specialized forms to complete your transaction and/or business; liquor licensing is no exception.

Individual/Seperate Forms

DLC1504 Solicitor Registration.
This form must be completed and sent with a check made payable to the Ohio Division of Liquor Control if you wish to act as a salesperson/representative of an Ohio manufacturer, Ohio distributor, out-of-state supplier, vendor or broker.

DLC1511 Application For Label Registration.
All Beer, Wine and Mixed Beverages must be registered in order to sell these products in the State of Ohio.
This form must be submitted to the Beer & Wine Section for proper product registration by the manufacturer or supplier.
This form must be completed and sent with a check made payable to the Ohio Division Of Liquor Control.

DLC1516 Ohio Excise and Use Tax Filing.
This form must be completed and sent with a check made payable to the Treasurer, State Of Ohio if you wish to import alcoholic beverages into the State Of Ohio.

DLC1536 Application For Tasting By A Manufacturer Or Distributor.
This form must be completed and sent to the Ohio Division Of Liquor Control, Attn: Beer & Wine Section.

DLC1539 Territory Designation Form.
When beer, wine and mixed beverages are registered to sell in OHIO, the manufacturor or supplier must designate an Ohio wholesaler to distribute these products. This form is used by the manufacturer or supplier to designate a wholesale distributor and assign exclusive territories to that distributor.

DLC1540 Quarterly Beer Sales Report.
This form must be completed every quarter and mailed to the Division by all A-1 permit holders.

DLC1541 Application For Tasting By A Manufacturer Or Liquor Broker/Supplier.
This form must be completed and sent to the Ohio Division Of Liquor Control, Attn: Beer & Wine Section.

DLC1551 Application For Supplier Registration.
A "supplier" is a company that sells alcoholic beverages in Ohio for resale at wholesale.
This form must be completed and sent to the Ohio Division Of Liquor Control, Attn: Beer & Wine Section.

DLC1608 Solicitor Renewal.
This form must be completed and sent with a check made payable to the Ohio Division Of Liquor Control if you wish to continue selling alcoholic beverages in the State of Ohio. This registration/renewal is once every two years on your birthday.

DLC1611 F-8 Permit Application.
This "F-8" form is for a Temporary permit to a not-for-profit organization for sales on publicly owned property located in Hamilton County ONLY.

DLC1614 Sale of Wine to a personal customer via mail order.
An S permit may be issued to a wine manufacturer, a brand owner or United States importer of wine, or the designated agent of a brand owner or importer for all wine sold in Ohio for the owner or importer to sell wine directly to consumers via the mail provided the manufacturer of the wine is entitled to a tax credit under 27 C.F.R. 24.278 and produces less than one hundred fifty thousand gallons of wine per year.

DLC1615 Sale of Wine to a retail permit holder.
A B-2a permit may be issued to a wine manufacturer, a brand owner or United States importer of wine, or the designated agent of a brand owner or importer for all wine sold in Ohio for the owner or importer to sell wine to retail permit holders provided the manufacturer of the wine is entitled to a tax credit under 27 C.F.R. 24.278 and produces less than one hundred fifty thousand gallons of wine per year.

DLC4029 Non Profit Entity Disclosure.
This form must accompany all "Transfer of Ownership", "Transfer of Location", "D7 Applications", "D5B Applications", "B Applications", "A1A Applications", "A Applications" and "New Permit Applications"; If you are a non profit businesses, municipal corporation or an educational institution organized not for profit.

DLC4030 For Corporate Applicants.
This form must accompany all "NEW" and "TRANSFER" applications.

DLC4031 For Partnership Business Applicants.
This form must accompany all applications of a partnership. It is used with all "NEW" and "TRANSFER" applications.

DLC4032. For Limited Liability Company Business Applicants.
This form must accompany all "NEW" and "TRANSFER" applications.

DLC4050 Club Officers Statement.
This form must be completed and sent with your renewal of D4 (private clubs) permits only.

DLC4070 Physical Inspection.
This defines "some" of the basic requirements for the physical inspection of a permit premises. A permit premises must pass a formal inspection "before" it receives a Liquor Permit.

DLC4079 Application for "E" permit.
Permit "E" may be issued to the owner or operator of any railroad, a sleeping car company operating dining cars, buffet cars, lounge cars, or similar equipment, or an airline providing charter or regularly scheduled aircraft transporation service with dining, buffet, club, lounge, or similar facilities, to sell beer or any intoxicating liquor in any such car or aircraft to bona fide passengers at retail in glass and from the container for consumption in such car or aircraft, including sale on Sunday between the hours of one p.m. and midnight.

DLC4080 Application for "G" permit.
Issued to owners of drugstores and for the sale of industrial alcohol for mechanical, chemical or scientific purposes.

DLC4085 Summary of Tenancy Rights.
This document is needed when the applicant does not own the property. This shows who does own the property and that they have given you permission to use the property. This form must be completed and filed with "NEW" and "TRANSFER" applications.

DLC4096 Financial Verification.
This document is to show the funds used to "set up" the business. This form must be completed and filed with "NEW" and "TRANSFER" applications.

DLC4113 "NEW" Permit Application.
This is the Application that must be filled out and filed when applying for a "NEW" Liquor Permit.

DLC4115 F Permit Application.
An "F" permit is a "temporary" permit for a Five-Day privilege for special functions. It is valid for the sale of BEER ONLY, until 1:00am. Filing fee is $40.00 and application must be filed at least fifteen days PRIOR to the date of function.

DLC4118 "TRANSFER" Permit Application of Location (only).
This is the Application that must be filled out and filed when applying for a "TRANSFER" of Location (only) of a Liquor Permit.

DLC4120 "TRANSFER" Permit Application.
This is the Application that must be filled out and filed when applying for a "TRANSFER" of ownership and/or location of a Liquor Permit.

DLC4121 Personal History Background.
This form is needed in order to conduct a background check of applicant(s) by the local law enforcement authority. It is a required form for all "new" and "transfer" applications. You must complete this form and mail to the Division of liquor Control.
DO NOT MAIL TO THE LOCAL LAW ENFORCEMENT AUTHORITY.

DLC4124 Application for D-7 Resort Area Permit.
Beer and intoxicating liquor by glass or container, for consumption on premises only, until 2:30 a.m. during the resort season. Permit Fee: $469.00 per month each month of the resort season. Payment for full resort season must accompany this application.

DLC4137Application for D-5B Enclosed Shopping Mall Permit.


DLC4138 F-2 Permit Application.
The "F-2" permit is for a function sponsored by a nonprofit organization organized for a charitable, cultural, fraternal or educational purpose, to sell beer, wine, mixed beverages, and spirituous liquor by the drink at a place where the sale of beer, wine, mixed beverages, and spirituous liquor on that day is otherwise permitted by law. The applicant-organization shall not be affiliated with the holder of any permit other than a D-4 permit, but a D-4 holder cannot be issued an "F-2" in its own name. An "F-2" permit is effective for not more than forty-eight (48) consecutive hours, and no sales can be made after 1:00am. No more than two "F-2" permits can be issued to the same applicant in one calendar year. Filing fee is $150.00; $160.00 if issued jointly and application must be filed at least twenty days PRIOR to the date of function.

DLC4158 Application For "Stock Transfer Consent "with instructions.

DLC4171 Application for D-4 (Club) Permit.
This form must be filled out and submitted together with documents referred to in section A and section B of the General Instruction accompanying form DLC4171.

DLC4174 Application For "A" Manufacturers Permit.

DLC4176 Application For "B" Distributor Permit.

This form is to be used for the filing of a D5A class only.

DLC4258 Duplicate Bar Application ORC 4303.30
§ 4303.30 Duplicate permits.
The rights granted by any D-2, D-3, D-3a, D-4, D-4a, D-5, D-5a, D-5b, D-5e, D-5f, D-5g, D-5h, D-5i, D-5j, D-5k, or D-6 permit shall be exercised at not more than two fixed counters, commonly known as bars, in rooms or places on the permit premises, where beer, mixed beverages, wine, or spirituous liquor is sold to the public for consumption on the premises. For each additional fixed counter on the permit premises where those beverages are sold for consumption on the premises, the permit holder shall obtain a duplicate D-2, D-3, D-3a, D-4, D-4a, D-5, D-5a, D-5b, D-5e, D-5f, D-5g, D-5h, D-5i, D-5j, D-5k, or D-6 permit.

The holder of any D-2, D-3, D-3a, D-4, D-4a, D-5, D-5a, D-5b, D-5e, D-5f, D-5g, D-5h, D-5i, D-5j, D-5k, or D-6 permit shall be granted, upon application to the division of liquor control, a duplicate D-2, D-3, D-3a, D-4, D-4a, D-5, D-5a, D-5b, D-5e, D-5f, D-5g, D-5h, D-5i, D-5j, D-5k, or D-6 permit for each additional fixed counter on the permit premises at which beer, mixed beverages, wine, or spirituous liquor is sold for consumption on the premises, provided the application is made in the same manner as an application for an original permit. ... The application shall identify by name, or otherwise amply describe, the room or place on the premises where the duplicate permit is to be operative....

Note: The holder of a D-1, D2X, or D-3X permit is not required to obtain a duplicate bar permit if the additional bar is exclusively used for the sale of beer only, or wine only if a D-3X. An A-1-A duplicate bar permit is only required with a D6
